Find the services you need with the Care Circle Connect agent

Finding dementia-friendly services and support can be confusing, and this is especially hard for long-distance caregivers. Our new application is taking the stress out of building the care circle at every stage of the dementia journey, whether you live near or far the person you're supporting.

Building with evidence

We're applying the principles from research on cognitive rehabiliation and reablement (CRR) in the ActivPrompt and Care Circle Connect applications.

CRR focuses on improving functional independence and quality of life; this means providing the right support to help individuals do everyday tasks better, feel more confident, and stay independent at home longer.

We've pulled together important findings from the World Alzheimer Report and Cochrane Reviews trusted by experts to help you understand the evidence behind our work.
